Event Details
The Chronic Disease Self-Management Program, originally developed at Stanford University, is a free educational program presented by the Community Council of Greater Dallas and the Area Agency on Aging of Dallas County.
The program includes six 2 1/2 hour classes for adults experiencing chronic health conditions such as hypertension, arthritis, heart disease, stroke, lung disease, chronic pain or diabetes.
